Chastidy Mader is a board-certified Family Nurse Practitioner and the founder of Haven’s Holistic Healing. She provides compassionate, whole-person care rooted in the belief that health is more than the absence of illness — it is physical, emotional, and mental wellness working together. Through her practice, Chastidy offers primary care, medication management, wellness support, weight-management care, hormone therapy, aesthetics, and recovery-focused treatment, including MAT services. Her approach is personal, judgment-free, and centered on meeting patients where they are while helping them move toward lasting stability, confidence, and healing. At Haven’s Holistic Healing, Chastidy’s mission is simple: to provide care that restores hope, supports healing, and treats every person with dignity.

As a Certified Peer Support Professional (CPSP) with more than 25 years of lived experience with addiction, bipolar disorder, PTSD, anxiety, and incarceration, I bring authentic understanding and hope to every interaction. My personal journey has taught me the realities of recovery, and I am deeply committed to supporting others through a compassionate, non-judgmental, and harm reduction-informed approach. I believe in meeting people exactly where they are—no matter their current stage or circumstances—and walking alongside them with respect and empathy. Using a continuity case management model, I partner with individuals to explore every available resource, coordinate care, and create a consistent, supportive experience from start to finish. Together, we focus on practical strategies that reduce harm, build stability, and move toward the recovery goals that matter most to you. I am passionate about empowering people to take ownership of their well-being while providing steady guidance and advocacy. I am also actively pursuing continuing education to strengthen my skills and expand my credentials so I can serve you more effectively. My approach is rooted in compassion, collaboration, and the firm belief that recovery is possible—and that no one has to walk the path alone.

Montina Herbert holds a Master’s Degree in Psychology and is a Licensed Addictions Counselor (LAC), CADAC II, and DBT-trained clinician. Since 2015, she has helped individuals and families navigate addiction, mental health challenges, and recovery. Her experience includes serving as Director of the Recovery While Incarcerated (RWI) Program within the Indiana prison system and providing services for children and families through the Department of Child Services. She is currently pursuing her LMHCA credential to further expand her clinical expertise. Montina takes a strengths-based, compassionate approach, creating a safe and supportive environment where clients feel empowered to achieve lasting change and personal growth. She is the owner of Therapy’s Peace Within Recovery Counseling and also provides counseling services through Haven’s Holistic Healing, offering both in-person and telehealth appointments. Jane is a healthcare and recovery home strategist committed to building safe, structured, and compassionate environments for all individuals. After years of witnessing the gaps and limitations within the healthcare system, she and her mother chose to step forward and create the kind of support they wished existed. Open Heart Homes was born from that shared conviction—a belief that every person deserves stability, dignity, and a place to heal. Jane and Open Heart Homes, partnered with Havens Holistic Healing, are dedicated to offering spaces where people feel seen, supported, and genuinely cared for as they rebuild their lives.

My name is Stephanie, and I am a Certified Peer Support Professional with lived experience in addiction recovery, incarceration, domestic violence, childhood abuse, sexual assault, mental health challenges, grief, and loss. I was also raised in a family impacted by serious mental illness, which contributed to a deeply unstable and traumatic upbringing. As an adult, I experienced domestic violence and struggled with the long-term effects of trauma, including anxiety, depression, and substance use. My journey eventually led to incarceration, where I began the deeper work of healing and rebuilding my life. Today, I live in recovery and use my lived experience to support others facing similar challenges. I understand the complexity of trauma and addiction, as well as the strength it takes to begin again. My approach is rooted in empathy, respect, and the belief that healing is possible for everyone. As a CPSP, I work alongside individuals to help them build stability, develop coping skills, and move toward a life of purpose, dignity, and hope.

Ryan Mader serves as a Peer Recovery Coach with Haven’s Holistic Healing, bringing lived experience, professional training, and a strong commitment to helping others rebuild their lives. After overcoming 26 years of addiction, Ryan uses his own journey of recovery, re-entry, faith, and personal transformation to connect with individuals in a way that is honest, practical, and grounded in real life. Ryan holds an ACIT credential and has completed approximately 4,200 hours toward CADAC certification. He is also pursuing a master’s degree in Human Services and Law, with the goal of strengthening his ability to advocate for individuals, families, and communities impacted by addiction, incarceration, trauma, and instability. At Haven’s Holistic Healing, Ryan specializes in helping people in recovery and those re-entering society after incarceration find the resources, support, and direction they need. His work focuses on connecting individuals to recovery supports, housing resources, employment pathways, community services, accountability, and hope. Ryan’s mission is to help others understand that no matter how long they have struggled, change is possible. His approach is direct, compassionate, and rooted in dignity, offering support to those who are ready to rebuild their lives one step at a time.

Meet Kylie Miller, LCSW, co-founder of Elm Counseling Services.
Kylie has worked in mental health settings since 2019 and has been providing outpatient therapy since 2021. She also spent a year and a half in palliative care, supporting patients and families through complex emotional experiences, life transitions, grief, and uncertainty.
Kylie specializes in anxiety, depression, trauma, OCD, and burnout, with an additional focus on perinatal mood disorders. She is currently completing certification training in both perinatal mental health and EMDR, allowing her to continue expanding the depth of care and support she provides. Known for her warm, patient-centered approach,
Kylie incorporates CBT, motivational interviewing, and supportive therapy techniques to help clients feel heard, understood, and empowered in their healing process.
