General Health / Primary Care
Primary care with Chastidy Mader, FNP-BC covers wellness visits, preventive care, and ongoing management of chronic conditions, along with medication management, weight-management care, hormone therapy, and aesthetics. Care plans are built collaboratively and adjusted over time as your needs change. Most insurance, HSA, and FSA health plans are accepted.

Addiction Treatment and Recovery Support
Support for substance use is approached without judgment. Care includes recovery-focused treatment and MAT services with Chastidy Mader, FNP-BC, alongside Certified Peer Support Professionals and Peer Recovery Coaches with decades of lived experience. Our team helps connect individuals to recovery supports, housing resources through Open Heart Homes, employment pathways, community services, accountability, and hope — including support for those re-entering society after incarceration.

Therapy Services
Therapy is provided in partnership with Kylie Miller, LCSW, co-founder of Elm Counseling Services. Kylie specializes in anxiety, depression, trauma, OCD, and burnout, with an additional focus on perinatal mood disorders, and draws on CBT, motivational interviewing, and supportive therapy techniques in a warm, patient-centered approach.

Psychiatric Services
Mental health visits include a careful evaluation, a clear conversation about options — including whether medication may be appropriate for you — and regular follow-up to review how things are going. Care is trauma-informed and coordinated with therapy and peer support when helpful.
